Homeless Youth Program

Service Description

Connect Health & Community runs a program for Innovative Health Services For Homeless Youth (IHSHY), and has an Outreach Health Worker for homeless young people in Cities of Bayside, Kingston and Glen Eira. Services provided include:

  • Counselling young people and families when appropriate.
  • Casework, support, linking and referral to other health and welfare services as needed.
  • Health Promotion Projects and activities such as:Secondary consultation
  • Homeless Health Promotion Working Group
  • Young Mothers' Group
  • Health information, education and training

Eligibility

Homeless and at risk young people up to 25 years in municipalities of Bayside, Glen Eira and Kingston.

Sites Providing Service

This service can be accessed from our Bentleigh & Sandringham sites.

Outreach to various locations e.g. Youth refuge, schools, youth centres, youth housing, other community settings if appropriate or needed.
